I would go B and D, https://devcenter.heroku.com/articles/writing-data-to-salesforce-with-heroku-connect, "When synchronizing changes from Salesforce, Heroku Connect will try to match records by their Salesforce ID (if present) and will fallback to the Unique External Identifier if it is configured and populated. However, if no Unique External Identifier is configured or populated and there is no matching Salesforce ID, then Connect will create a new record in Postgres (in this case, a duplicate record)" I understand then that an External Identifier is not required for bidirectional synchronization but recommended.
