BD. Add only will create duplicate if the record already exists

Not if you set the primary key it won't. "Add only - The import creates records for primary keys that do not exist in the data extension and ignores records whose primary keys exist." (https://help.salesforce.com/s/articleView?id=sf.mc_es_import_into_de.htm&type=5)

Yes, but since the question is framed such that we assume the two ways need to be exclusive, BD makes more sense because even if there is no Primary Key set, Add and Update will take care of the duplicate records
