A is correct but it is not within the checkpoint report the Project Manager captures an informal issue in the highlight report that is given to the Project Board which is noted in the lessons log.

A is correct A.14.3 Derivation The lessons log is derived from the following: ● lessons from other projects ● project mandate or project brief ● daily log, issue register, quality register and risk register ● checkpoint reports and highlight reports ● completed work packages ● stage plans with actuals ● observation and experience of the project’s processes.
