Correct answer should be A - page 150: "During execution of a stage plan, if any forecasts indicate that the management stage is likely to exceed the agreed management stage tolerances, then the deviation should be referred to the project board by the project manager in order to get a decision on corrective action. This would be done by raising an issue and an exception report."

I think the question is very confusing. It states that the checkpoint report is late, not that the product delivery is late. The project manager just did not receive information. This is not a situation to raise an exception report. I think D is the correct answer.
