This is wrong. I think the correct answer is B. off-specification according to the definitions in the Prince2 manual. off-specification (matches the question) Something that should be provided by the project, but currently is not (or is forecast not to be). It might be a missing product or a product not meeting its specifications. It is one type of issue. request for change A proposal for a change to a baseline. It is a type of issue. problem A type of issue (other than a request for change or off-specification) that the project manager needs to resolve or escalate. Also known as a concern.

Again, why is C highlighted as correct? Is anyone monitoring this site?

B; The other options are not the most appropriate in this context: A. A request for change: This is used when a change to the product is desired, not when a product fails to meet existing criteria. C. A problem: While a problem might be the cause of the off-specification, the off-specification itself is the formal issue. D. A concern: This is a less formal term and doesn't trigger the same level of control and management as an off-specification. In summary: When a quality review finds that a product doesn't meet its defined criteria, an off-specification should be raised to formally record the issue and trigger the appropriate corrective actions.

Definitively the answer is B. Reference: Off-Specification is something that should be provided by the project, but currently is not (or is forecast not to be). It might be a missing product or a product not meeting its specifications.
