You are correct, but where does it states' must be written' , Id assume D is then correct. Prince2 is doing some mumbojumbo but better to learn the ' Prince' way for exams immo, they use confusing terms tbh.

Minimum requirements
You must define your project's organization structure and roles (by ensuring all responsibilities in PRINCE2’s role descriptions are fulfilled). You must define your project's approach to communicating and engaging with stakeholders and you must produce and maintain the project initiation documentation (PID), and a communication management approach.
It does not say anything on "written" (key word) role descriptions.
That is correct as in chapter 7.2, but why do you think that "The ->FREQUENCY of communication<- to the project's stakeholders must be documented" is an approach/a way for doing the "communication and engaging"? I am not a native speaker, so perhaps that is why D does not make any sense for me, and would picked A.
