I think both are correct:
C: The project (under the Change theme) should define how product baselines are created, maintained and controlled.
D: Ensure that issues are captured, examined, managed and reviewed throughout the project
C is the correct answer.
It cannot be D because only the formal issues are recorded in the issue register. The informal, are registered in the daily log.
C and D are correct according to the PRINCE2 manual, section 11.2:
• define how product baselines are created, maintained and controlled
• maintain some form of issue register to record identified issues and decisions relating to their analysis, management and review
• ensure that project issues are captured, examined, managed and reviewed throughout the project lifecycle
D is the right answer.

Minimum requirements
You must define a risk management approach covering the risk management procedure to be used, and the roles and responsibilities for risk management. You must maintain some form of risk register to record and manage risks and must ensure that risks are identified, assessed, managed and reviewed throughout the project. You must use lessons to inform risk identification and management.
C is correct. D says to maintain the issue register, not to record every issue there. B is incorrect too, as the change authority does not have to be the PM
But D is also correct as it says "maintain some form of issue register to record identified issues and decisions ..."
