The quality register is updated to reflect (in summary form) the actual results from the quality activities. The quality register provides key audit and assurance information, relating what was planned and agreed (in the quality management approach and product descriptions) to the quality activities actually performed . prince2 guide, 8.2 C should be correct

The correct answer is C. Since it has nothing to do with "issues". https://prince2.wiki/management-products/quality-register/ I like to say that a Quality Register does three things: Provides an overview of the planned quality activities and provides a unique reference for each quality activity. Provides an overview on the status of all quality activities and a pointer to the quality records. And at the end of the project, it provides a history of all quality activities that have taken place and all results.
