in question it is stated as an issue, so it is not a risk, option A gets eliminated there. Option D mentions reviewing the quality process which is better option

Question says" Known issues". Known issues are nothing but " Risks". An unknown or unidentified risks if it occurs is called an issue. Moreover there are production issues, that may not be necessarily quality issues. Content of the question nowhere states that there is issue with quality per se. So option A is the best fit...

I tend to D, the Question did not mention anything about risks. although if there are known issues in production, that means that the QA process is flaw.

Known issues in production could pertain to Schedule, cost or procurement . Not necessarily quality unless stated otherwise in the content of the question.
