A very confusing question. The choice is clearly between A & D.
D may be considered as less correct because you don't report (a problem & rectifying actions) to CCB but you make a request (for change). On the other hand, the fact that "rectifying actions" is included, this may be considered as a request for action with the problem reporting being the justification for the rectifying actions that need to be taken which makes case for D.
A may be considered as being correct because the PM is reporting the problem and also proactively recommending rectifying actions. However, reporting to the PMO does not necessarily solve the problem but can only lead to steps towards correcting the problem.
I would lean towards D because it's the ultimate step that has to be taken to correct the reported problem.
It looks like the reason of not A is because there is rectifying actions and if there is rectifying actions, that means there is change, so the first action should be reporting to CCB. Not sure if my understanding is correct.
