I think D is correct. Sponsors keep asking for more features. Option "B" does not fix the problem, team cannot find a solution to the problem. Even defining DOD would not help. DOD simply is just an acceptance criteria for the deliverable. Defining DOD will not stop the stakeholders from asking more. We need a project charter to define the project objectives and high level requirements.

I will go for C as the problem is lack of definition of done of the requirements and not lack of directions and vision. the it's the product owner responsibility

I think D is correct. Because: The project manager realizes that due to the lack of a "clear definition of project objectives" and definition of done (DoD) -> So that I think need to build the agile project charter and set clear expectations for the project with key stakeholders in the meeting.
