the reason for A is if a user story is mentioned not on the board and mentioned by the team member, that may imply an issue or dependancy to the current Sprint and has not been identified yet until now. Since the question was asking what scenarios are 'problems', I think PM should be pay attention to this option A.
I think D is not a problem as reviewing the backlog although it is not best practice ,,but not really a problem to PM
