Here is my thinking, if you (PM) change the scope then, Scope creep is the term used to describe the gradual expansion of the project scope beyond the original agreement, often without formal approval or adjustment of the budget, schedule, or resources. Change requests are the formal or informal requests from the stakeholders or clients to modify the project scope, deliverable, or requirements. so i believe its A since all the stakeholders(team) agreed on missing deliverable and needed to add this deliverable in their scope.

As long as it’s a traditional approach, you can’t make any changes to the scope without a change request.

its not a risk , its an issue. Think about that you are working on a constructing a house and you missed one key deliverable i.e. car poarch, what you gonna do? look for home map ( risk management plan) or you need to revamp your work with the approval of builders ( change request) ?

"A" makes more sense to me. The second best option is the risk management plan. Risk management plan helps you identify and manage identified and unidentified risks. This situation is an unidentified risk, the risk management plan will help you with how to manage unidentified risks. But since this is a key deliverable and we already know the impact on the critical path, the best path is to just submit the change request as soon as possible and then discuss it with the CCB.
