A&B. and for what its worth, I don't think a "backlog register" is a real thing. The change log is irrelevant because you haven't made any changes.

will go with AD. because you dont need to register a risk so quickly. so will go with issue log and change log A. Issue log: The issue log is used to track and manage project issues, including any problems or errors encountered during the project. I think it should be AB. You update the risk Register by removing the realized risk into the issue log.

D would have been true but the question says which docs need to be updated as soon as possible. Change log would be updated after the change request has been raised. Risk log would be updated and the remediation plan in it would be to create a change request and then it goes into change log. So, my answer is B instead of D
