Would go with B. Mitigation actions were not executed in a timely manner. This means mitigations actions were done but not on timely manner. So the Risks should have been monitored correctly and based on the outcome and impact mitigation should have been done on timely manner. C says - Identified the appropriate response. It never said in the scenario that response was applied inappropriately so cannot be an option

B: Monitoring & Controlling the issue The question states that "mitigation actions were not executed in a timely manner", I will assume that at some point, the project manager had carried out option: "C: Identified the appropriate response and had it implemented" but not in a timely manner which leaves room to say the PM might have not taking a close look during the Monitoring and Controlling phase of the project leaving option "C: Identified the appropriate response and had it implemented" for the very last minute which might lead to the project stakeholders frustration and the delay of the project.

C - The key word is mitigated. We mitigate, RISK. The question is about risk. The proper risk response would have prevented this situation.
