There are two problems to be addressed in this question: 1. less defects 2. less change requests Defect - Customer is not happy with the results, feature(s) not working as expected Change Request - features working as requested but the customer wants something more Answer C, change the control mechanism may solve the first issue (defects) but will not decrease the number of change requests. I think it should be D, increased opportunities to confirm common understanding with the client should solve both problems. Common understanding = team knows what customer expectations are so they develop exactly what customer needs (less change requests) and with good quality (good definition of done). I vote D.
