I think it should be A. D - The tem already cannot solve their impediments themselves. C - Is it necessary to follow "all" improvement actions? It depends what actions they are, and how much they can improve.

The SM is a facilitator - he helps the team as a servant leader, showing how should the team behave in order to resolve these kinds of impediments (as described) by themselves (since these impediments are not show stoppers - the team knows how to solve them, but there is lack of initiative). Thus the most logical option is D. It includes A and includes C (B is not an option at all).
