C is the correct answer, D is reactive. While having a process is good, that wont' stop the customer from requesting a ton of changes. If you have a tight scope statement You don't have to go through all the change request crap.

I selected C because the PM was warned. He should review the lesson learned because the scope statement has a boundary to exclude the not in scope items. The scope baseline should be approved. Once the scope baseline is approved, any change should be via the agreed integrated change control procedures. Based on the lesson learned, we should set project boundary as the first step, set change control is the second step
