People. please stop using AI-generated answers as a reference. They are a complete fantasy, that is not called research. For example this one, it seems like a solid answer, but it's totally made up. There are no such controls, mentioned in the referenced page, in Microsoft Forms. You cannot embed canvas app in a form. So the answer is No, No, Yes.

There is no way you can embed anything, let alone a canvas app, into a Microsoft Form. Microsoft Forms are very mundane and lack even basic features.
