The answer is A & C in the question it states that "You plan to automate the deployment of a virtual machine scale set that uses the Windows Server 2016 Datacenter image" D defeats the purpose of the automation requirement, so C is 100% correct. Install an app with the Custom Script Extension The Custom Script Extension downloads and executes scripts on Azure VMs. This extension is useful for post deployment configuration, software installation, or any other configuration / management task. nothing in the question points at automation beyond this particular scale set. you just want to have iis on every vm that the scale set spins up. i think you make it way too complex with options like github for this purpose.

Since you use the the ARM template to create the scale set, you don't need to create it as a separate task, so D is not required

In my opinion A and C are correct, and C and D are also correct. To use a custom script, you can point to a URL on GitHub for example, or point to a Storage Account, where you will need to upload the script. Just find out now, what Microsoft wants to "hear".

A and C to me When you define a virtual machine scale set with an Azure template, the Microsoft.Compute/virtualMachineScaleSets resource provider can include a section on extensions. The extensionsProfile details what is applied to the VM instances in a scale set. The answer should be C and D https://docs.microsoft.com/en-us/azure/virtual-machine-scale-sets/tutorial-install-apps-template#create-custom-script-extension-definition A. Where would you upload the script? B. Azure policy won't help in Installation of IIS. C. Modifying the ARM template give you flexibility to change the extension profile. D. You can still use Azure Portal to "Deploy a custom template". E. Creation of automation account won't help with IIS installation on VM. If the question was about a single VM only, then you should have selected "Extensions" and used "Custom Data" to write a script.
