No,Yes,Yes Its model or canvas. And here source is SharePoint, so canvas. They should download the app, because that gives easy access on mobile to the canvas app.

The app will be created for a mobile hence a phone format which although can be seen in a browser it won't be formatted correctly so I would say YES for the third question.

The app can be used from a web browser on a phone, but Microsoft recommends for the best experience and full set of capabilities on your mobile devices, the Apps Mobile app. It would really be something optional and not mandatory

Last option should be NO. Users can also go to the SP site using any mobile browser, and upload a status report. The mobile PowerApps app is NOT required, but optional and more convenient.

Model-driven apps are built on Dataverse, and can only connect to a Dataverse environment. All the data that defines a model-driven app is stored within Dataverse.
