This question has very bad answers. Not cause the answers are bad in theory but it can be construed in various ways. "A" seems like the best step, then "B" to follow. However, I can see B being valid because chances are you want to prevent this issue in the future. By the PM adding the stakeholder to the distribution list first then he will not have to worry about this being an issue in the future or possibly getting distracted and forgetting to add the stakeholder to the list. You can always add the stakeholder to the list then send the weekly report.

I'm not entirely convinced the answer is D, I can see the case for it, but it says "consult the communications plan and act accordingly" which seems to suggest the project manager simply looks at the comm plan and does what it says rather than what the stakeholder is actually interested in. Would the PM go back to the stakeholder and say, "Sorry, you're not included in the comm plan, you're out of luck."
I could be looking at it incorrectly but thought I'd add food for thought in case some others have better insight.
