Given answer looks fine. It is stated that the PM lets the stakeholder know about the change and the sponsor agreed. That rules out B and D. C wouldn't make sense cause the project sponsor would've been involved in the project processes. My thinking is project over budget due to vendor>new vendor found> sponsor notified and approved change > budget back in target parameters > deliverables arrive and sponsors not happy. This leads me to believe the new vendor provided low quality material.

This has to be B - Did not follow the communication plan - leading to upset stakeholders. It says the change was approved and it seems the only people upset with it are those that were not informed of the change.

This one is confusing. I see the answer could be quality because of less expensive materials however, it doesn't state they are using a lower grade, just a different vendor. So, this is unknown. Most of all, I see D. The change was not documented and approved This seems more plausible since the question states the PM got approval from the project sponsor and not the CCB. The sponsor might approve of the change but the change still needs to go through the change management process and the CCB needs to approve the change and the change needs to be documented and communicated.

It says the project came within the target tolerances, meaning quality did not suffer. I honestly think the answer is that the PM did NOT adhere to the Communications plan, letting key stakeholders know of the change, this would explain their frustration. It says the change was approved by the project sponsor.
