I would say D. Waterfall itself isn't very flexibel. However if changes are carried out before the project start this would be a flexible way to perform the changes without rework during the project phase.

A may be correct for extra effort and time to review, approve new change and redo documentation. Significant or not depend on how large the change. B is wrong as change raised before first step. No implemenation work did before except documentation. C always correct. The main point is the occur timing. This question is at very early stage and should be the best timing. D is wrong as no flexible for Waterfall model, all steps depend on previous step.

Redo work is expected for whole project, but the situation of the question is more focus on very early stage. Waterfall can require that a phase be fully completed before proceeding to the next one, and it’s extremely difficult to go back if you miss anything. That means you and your team must check every box before moving on, or there could be a lot of rework needed.
