Looking at this question it tells us there is 10GB and 40GB modules installed, I'm pretty sure this is not a supported configuration.

Port 1 in Slot 1 and Port 2 in Slot 1 When using Dual MHO's Odd are connected to SLOT1 (MHO), and evens are connected to SLOT2 (MHO) I pretty sure you should not be connected port 2 to SLOT1 in a dual MHO configuration.
