Number A is the most complete answer. It includes the client, the representative, and the interdisciplinary team. The client and representatives have the right to know about the intervention you are implementing with them. In the end, they are the ones who receive the treatment and consent to it.

No because client/consumer consent of goals is always going to be the correct answer in ethical questions like this
