A. R91.8, C56.9, C78.7 is incorrect that concludes abnormal finding code R91.8 IS UNNECESS HERE SINCE WE HAVE SPECIFIC DIAGNOSIS. IT SHOULD. BE. B C56 C787 C7802

If there is no pathology report to confirm the diagnosis then A is the answer, but because the pathology result is given therefore D is the correct answer.
